Let's start with the big picture. Youth mental health has not just edged downward since the mid-1990s; it has taken a real and measurable hit. This isn't a quirk of questionnaires or new diagnostic fashions.
As Rutter and Smith framed it a generation ago, adolescence now stretches into the mid-twenties, and across that elongated runway, the mix of stressors has changed: more family strain and sharper employment expectations, and less of the protective scaffolding that once buffered young people. Layer onto that the megatrends we're all living through — rising inequality, high-stakes schooling, digital platforms running twenty-four seven, climate anxiety, and economic precarity — and you get a compound risk profile. COVID didn't create it; it poured accelerant on it.
The age window matters. Adolescence through emerging adulthood concentrates onset for mood, anxiety, and related internalising disorders right when education, work, housing, and identity decisions stack up. That concentration shows up in costs too: lost productivity, stalled education, and service needs that pile onto under-resourced systems.
You can hear it in the headline numbers from Australia alone. Over about fifteen years, twelve month mental disorder prevalence in young people rose by roughly fifty percent, with the steepest rise among females. Affective disorders in sixteen to twenty-four-year-olds went from six point three percent in two thousand seven to thirteen point six percent in two thousand twenty to two thousand twenty-one.
In twenty-five to thirty-four-year-olds, it rose from seven point nine percent to eleven point zero percent. Depression among young adults more than doubled across roughly the last fourteen years. That's one country, and it mirrors what higher-income settings are reporting at scale.
So what exactly did the literature show? McGorry, Gunasiri, Mei, Rice, and Gao set out to answer that with a narrative review designed to track time trends rather than adjudicate single causes. They searched Medline back to nineteen forty-six, PsycINFO and Embase from near inception through May two thousand twenty-three, then extended in June two thousand twenty-four with additional terms to capture psychosis, eating disorders, and bipolar disorder.
After de-duplication, the May two thousand twenty-three search turned up twelve thousand forty-seven records, one hundred seventy-seven full texts, and seventy-two studies that met inclusion. June two thousand twenty-four added three hundred seventy-three records, one hundred twenty-two full texts, and eleven more studies. Six additional papers came in through reference chasing and Google Scholar. The final tally was eighty-nine studies.
The inclusion rules were tight. Samples had to include young people aged twelve to twenty-five and report trends with at least one time point after nineteen ninety-five on outcomes like anxiety, depression, psychological distress or well-being, suicidality, eating disorders, schizophrenia, or mood disorders. Administrative and service-use datasets were excluded from the core synthesis, with the focus on population measures.
Two reviewers independently screened the records in overlapping windows. To synthesise, they classified trends as increasing, stable or mixed, or decreasing; built a cross-study time trend using each study's most recent observation year; and reported percentage changes where available. This was always framed as descriptive, not causal.
The geography leaned heavily toward high-income countries: thirty-three percent of studies in Europe, twenty-nine percent in North America, thirteen percent in the United Kingdom, seven percent in Australia, and a handful in Asia, New Zealand, South America, and Iran, with only four percent in low and middle-income countries. Most were primary studies, and three-quarters were nationally or geographically representative. That gives you breadth, but it also flags limits on generalisability.
Measures changed across cohorts, instruments weren't harmonised, and sampling frames shifted — all the usual comparability headaches when you stitch time together. The authors are candid about that.
Even with those caveats, the signal is emphatic. Roughly eight in ten studies, eighty percent, seventy-one of the eighty-nine, reported an increase in at least one outcome among anxiety, depression, distress, low well-being, or internalising symptoms. Broken down further, depressive symptoms or disorders rose in twenty-six of thirty-seven studies, emotional or internalising problems in twenty of twenty-seven, and anxiety in fifteen of eighteen.
When you look at the composite time trend assembled from the most recent years, you see steady worsening from the mid-to-late 1990s into the 2010s, an additional step up beginning around two thousand sixteen, and then another lift in two thousand twenty to two thousand twenty-two. The pandemic period aligns with further elevation in distress and depressive symptoms, but the curve was already bending long before lockdowns.
Magnitudes vary by country. For anxiety, the United States shows increases broadly in the twenty-nine to eighty-four percent range, Sweden in the seventy-seven to one hundred sixty-four percent range, and Australia around one hundred five percent. Canada comes in near forty-three percent, New Zealand about forty-eight percent, the United Kingdom roughly fifty-five to sixty-one percent, and Poland between ten and sixty-four percent depending on the series.
For depression, the United States ranges from eight to one hundred nineteen percent, Australia around eighty-seven, while Finland shows a more modest plus eighteen percent. Across Western Europe, North America, and Australasia, the direction is consistent even if the slope shifts case by case. If you want the upper bounds across the literature, the review notes severe psychological distress up to about a two hundred forty-two percent rise, anxiety as high as one hundred sixty-four percent, and depression and low well-being up to roughly one hundred nineteen percent, with severe depression around one hundred forty-five percent. Those are outliers, not medians, but they sketch the envelope of change.
There's heterogeneity globally. The two thousand nineteen Global Burden of Disease analysis estimated a two point two percent decline in anxiety incidence among ten to nineteen-year-olds from nineteen ninety to two thousand nineteen, largely driven by lower-income settings. That's reassuring in one slice of the world and a reminder that different structural contexts are pulling in different directions.
In the United Kingdom, eating disorder referrals nearly doubled in recent years. In the United States, college students' mental health service utilisation rose steadily from two thousand seven to two thousand seventeen. Canadian systems also report mounting youth needs. Everywhere, waiting lists and unmet demand are the throughline.
Who's been hit hardest? Gender and socioeconomic gradients jump out. Across anxiety, depression, and distress, increases tend to be larger in females, with widening gender gaps in internalising problems in systems that ramped up grading and testing.
Socioeconomic status amplifies the slope: several countries report especially steep rises among lower-income adolescents and a fourfold widening of emotional problems across income strata in the United Kingdom when you compare earlier to later cohorts. Within the Netherlands and Nordic samples, you see pronounced within-country variation tied to school-related stress and local context.
What's driving it? Think in layers. At the distal end are the megatrends — political economy shifts, inequality, climate change, platform dynamics, economic shocks.
Those forces express themselves proximally through families, schools, and peer networks. The review threads those pathways using Kirkbride's social determinants framing. Educational reforms that raise stakes on grades and exams are linked to student anxiety, with disproportionate impact on girls.
Family environments matter: maternal emotional problems have modest but real associations with adolescent emotional problems, and frequent parent-adolescent conflict tracks with rising distress. Classic bullying has declined in parts of Europe, but both bullying and cyberbullying remain risk factors, and cyberbullying trends are patchy by country. Social media and device use correlate with sleep disruption and have small average associations with well-being; in one decomposition, platforms explain only about four percent of the observed rise in depressive symptoms, which means they're part of the story but not the script.
Economic shocks bite harder. Across countries, a five percentage point increase in unemployment during the Great Recession was associated with roughly a thirty-five to fifty percent increase in clinically significant mental health problems among youth. Add housing insecurity, intergenerational inequality, and climate-related worry, and you get stacked risk.
The throughline isn't a single villain; it's accumulation under conditions of lower slack and thinner buffers.
As for study design, most of this evidence is observational and population-based. Associations are clear; causality is not. The authors lean into that uncertainty and argue for methods that can sharpen attribution: interrupted time series to separate secular drift from shocks, natural experiments and quasi-experiments when policy or platform changes create exogenous variation, multilevel models to parse context from composition, and structural equation or path models to quantify indirect effects.
All of that rides on better measurement harmonisation across cohorts and the discipline to triangulate.
What do we do with this? McGorry and colleagues land on a three-part plan: prevention, early intervention, and treatment innovation, tied together by reach, quality, and timeliness. On prevention, they argue for layered strategies — universal, selective, and indicated — aligned with the megatrends we just walked through.
Universal school-based mental health education can lift literacy and protective skills, but effects are often small or neutral, and some programs risk iatrogenic harms if poorly designed or delivered. Selective approaches that target high-risk groups — for example, LGBTQIA plus youth — and indicated approaches that catch emerging problems can deliver more impact. The protective levers are familiar but often underused: social connection, mentoring, community-based programs that build social-emotional skills, and attention to digital and climate stressors.
Prevention takes time, but it anchors the Mrazek–Haggerty model the authors use to frame the system: prevent when you can, intercept early when you can't, and sustain recovery.
Early intervention is the pivot. Integrated youth services — think headspace in Australia or integrated behavioral health in primary care — reduce friction by offering stigma-free, one-door access to mental health, substance use, and vocational supports. Early psychosis models have already shown that cutting the duration of untreated illness changes trajectories; the field is moving to transdiagnostic platforms that bring that logic to anxiety, depression, and comorbidity.
Digital tools extend reach with psychoeducation, peer support, and referrals, but the ambition is coordinated care across primary and specialty systems, not apps in a vacuum. Cost-effectiveness evidence increasingly supports early intervention as a best buy.
Treatment needs to catch up with demand. Youth-focused services remain under-resourced and overwhelmed. The authors argue for holistic, evidence-based care, recovery-oriented practice, and peer support, with continuity to prevent relapse.
They point to group-based psychosocial interventions for trauma in humanitarian settings and to the consistent benefits of social inclusion as concrete, scalable examples. The punchline is blunt: despite incremental expansions, prevalence and impact haven't fallen in twenty-five years. Marginal tweaks won't do it.
Strategic public investment and cross-sector coordination are required, and policy choices should be guided by rigorous economics — cost-benefit analysis and multi-criteria decision analysis — that weigh health, education, digital governance, and climate resilience against youth mental health outcomes.
What stands in the way? Four interlocking barriers. Financing shortfalls starve services that already run hot.
Workforce and capacity constraints make scaling new models hard. Structural fragmentation leaves gaps in the everyday circles of support — families, schools, communities — and between sectors that don't talk to each other. And data and evaluation systems are weak: measures aren't harmonised, trend surveillance is thin, and causal understanding is limited.
Broader political economy dynamics — privatisation without accountability, frayed safety nets — compound inequities and blunt reform.
The fix is less mystery than muscle. Sustained political commitment, cross-sector coordination, and scaling evidence-informed models like integrated primary care and headspace-style youth hubs. Build a monitoring and learning spine that links megatrend shifts to youth mental health with designs that can separate noise from signal: interrupted time series, cross-cohort comparisons, cluster and stepped-wedge trials embedded in real services.
Track routine outcomes, not just pilots. And use economic evaluation — including multi-criteria approaches — to help ministries of health, education, housing, and digital regulation row in the same direction.
Let me end on two grounded takeaways and one forward look. First, the rise in youth internalising problems is real, large in many high-income settings, and heterogeneous by place, gender, and socioeconomic status. The best estimate across eighty-nine studies is that most indicators worsened since the mid-1990s, with an extra shove since the mid-2010s and another during COVID.
Second, the drivers are layered. Educational pressure, family strain, sleep-disrupting platforms, bullying and cyberbullying, economic shocks, and climate anxiety each contribute a slice; none explains the pie. That's why single-factor debates — "It's all social media" — keep stalling.
Looking ahead, the methodological agenda is as important as the service agenda. Harmonise measures across cohorts. Embed quasi-experimental evaluations into policy changes and platform shifts.
Use multilevel and path models to estimate indirect effects and heterogeneity by gender and socioeconomic status. If we do that while expanding prevention, early intervention, and youth-specific care, we don't just describe a crisis; we start bending the curve.
