Deprivation amplification revisited; or, is it always true that poorer places have poorer access to resources for healthy diets and physical activity?
Here is an idea almost everyone finds convincing: if you are poor, and you also happen to live in a poor neighbourhood, your disadvantage compounds. You already can't afford a car, and now there is no supermarket within walking distance. You already can't afford a gym membership, and now there is no park nearby either. The environment piles on. It makes a hard situation harder. Sally Macintyre helped build that theory. She called it deprivation amplification. Then she looked at the data and found that the map didn't match the story. The concept has two intellectual ancestors worth knowing. The first is the distinction between compositional and contextual explanations of health: compositional refers to who lives somewhere — their age, income, class — while contextual refers to what the place itself is like. Most studies find that who you are predicts your health more than where you live, but where you live still matters. Area effects show up for heart disease mortality, depression, physical activity, diet, and smoking. The second ancestor is the inverse care law, a phrase coined to describe how good medical care tends to be least available where it's most needed. Macintyre extended that logic outward — to supermarkets, parks, recreation centres, the whole built environment.
The idea that poor places lack what people need to stay healthy gained traction in health policy partly because it was politically useful: it shifted blame away from individuals and toward modifiable environments. Change the place, not just the person. That's a powerful framing. But here's where the empirical ground starts to shift. When Macintyre and colleagues actually counted and mapped facilities in Glasgow across two socially contrasting localities, the results were mixed in ways the theory didn't predict. Large multiple supermarkets were more likely to be located in deprived neighbourhoods. When price differences existed, healthier foods tended to be slightly cheaper in the poorer areas. The poorer locality had three recreation centres and one sports centre. The richer locality had none of those. Regarding green space, the share of residents living within three hundred metres of a green space larger than two hectares rose steadily with deprivation — from thirty-three percent in the least deprived areas, to fifty-two percent in middle-deprivation areas, to sixty-one percent in the most deprived. That's not the story deprivation amplification predicts. But the picture flips when you look at health services. The richer locality had nearly twice as many community health clinics, three times as many general practices, twice as many general practitioners, three times as many dentists, four times as many opticians, and one and a half times as many pharmacies.
So some resources were more available in deprived areas. Others were concentrated in wealthier ones. The pattern depended entirely on which resource you were measuring. Fast food tells a similar story of inconsistency. In Glasgow, fast food outlets were not clustered in poorer residential neighbourhoods; they were concentrated in the central business district, the West End, retail parks, and along arterial roads. However, in Melbourne and New Orleans, studies found the opposite: poorer and minority neighbourhoods had greater exposure to fast food. In parts of England and Scotland, the density of major chain restaurants was higher in more deprived areas. There is no single global pattern. There are only local ones. Supermarket access shows the same fragmentation internationally. In Brisbane and Alameda County in California, there were minimal socioeconomic differences in food shopping infrastructure. The United Kingdom Competition Commission, after planning changes in the 1990s, found no evidence of lack of supermarket access in poorer areas. But in Mississippi, North Carolina, Maryland, and Minnesota, wealthier neighbourhoods had more supermarkets — and one review found four times as many supermarkets in white neighbourhoods compared to Black neighbourhoods. In metropolitan Detroit and Minneapolis–Saint Paul, the nearest supermarket was significantly further away in heavily African-American and the most impoverished neighbourhoods.
Physical activity facilities show the same heterogeneity. Outdoor playgrounds were more common in poorer areas in Amsterdam, Edmonton, Boston, and Perth. In Melbourne and parts of the Netherlands, there were no socioeconomic differences. In England, the density of physical activity resources was lower in more deprived areas. The honest summary is this: the relationship between neighbourhood deprivation and resource availability is not a universal law. It is an empirical question with a different answer depending on which resource you're counting, which country you're in, and when you're measuring. Macintyre is explicit about why this matters for policy. If governments design interventions premised on the assumption that poorer places consistently lack healthy resources, those interventions will sometimes miss the real problem entirely — or even target places where the assumed deficit doesn't exist. But Macintyre's argument goes further than just correcting the count. Even when a resource is physically present, she argues, that doesn't mean it functions as access. This is where the paper makes its subtler and perhaps more lasting contribution.
In Glasgow, a new large supermarket opened in a poor neighbourhood. Studies of that store suggested its main beneficiaries were shoppers from outside the area who switched to it — locals continued using smaller nearby shops or didn't perceive the supermarket as being designed for them. The facility was there. The access wasn't. The numbers said one thing; the behaviour said another. Safety and symbolic meaning shape this further. A piece of vacant land might enable informal play for some children and feel threatening to women jogging alone or to elderly residents. A woodland can be valued by some residents and considered frightening by others. The same physical space carries different meanings for different people, and those meanings determine whether a resource actually gets used. Quality adds another layer of complexity. In Glasgow, Macintyre found that some markers of service quality actually favoured the poorer locality — more general practices operated out of purpose-built accommodation, more general practitioners had qualified relatively recently, and more practices had attached community nurses and specialist clinics. The richer area had more practices. But the poorer area's practices were, in some respects, newer and better equipped. Raw counts hide this completely.
What this means for measurement is consequential. Researchers who rely on proximity metrics — how many supermarkets within a kilometre or how many parks within walking distance — are measuring something real but incomplete. The questions that matter are harder: Is this facility of usable quality? Do local residents perceive it as accessible and relevant to them? Who actually patronises it? The mismatch between objective presence and perceived access is not a methodological footnote. It may be where much of the action is. Macintyre's policy conclusion follows from all of this. Don't abandon environmental thinking about health — the environment clearly shapes diet, physical activity, and health outcomes. But ground that thinking in specific, local, current evidence rather than assumed patterns. A policy designed around a deficit that doesn't exist in a given place will waste resources at best and mislead at worst. The research agenda she points toward requires more than counting. It needs longitudinal designs that track how resource distributions change over time. It needs cross-national comparisons that make historical, zoning, and planning contexts legible. It needs studies that measure quality and perception alongside objective presence, and that examine how individual characteristics — income, mobility, tastes — interact with area characteristics rather than treating those two things as independent forces.
The tension Macintyre leaves on the table is genuine and still unresolved. The environment matters for health. That part the evidence supports. But we do not yet have a reliable, generalisable map of how it matters, where it matters, and for whom. Deprivation amplification may describe some cities in some countries at some historical moments very well. It may describe others poorly or not at all. The theory captured something real. The data just turned out to be more complicated than the theory assumed. That's not a failure of the idea.
